Activating your online account

At PuntP we use Minddistrict as our digital care platform. You can access it on a computer, tablet, or using the app on your phone. If you are a client at PuntP, you will receive an invitation to join Minddistrict automatically. You can read more about this on the Minddistrict information page. In Minddistrict, you will also be able to complete the digital pre‑intake: this questionnaire helps you prepare for your intake. It is extremely helpful for both you and us if you fill in the questionnaire. This way, we have enough time to focus on your  personal story during the intake.

Completing the questionnaire before the intake 

We provide two questionnaires before the intake to help us understand your symptoms as accurately as possible. These questionnaires will be sent securely to your email address within 24 hours of scheduling your intake, with PuntP as the sender. If you haven’t received the email, please check your spam folder. Once you have completed the online questionnaires, the results will be added to your file automatically.
